Immunizations

The Dubois County Health Department Immunization Program operates in conjunction with the Indiana State Department of Health (ISDH).  All immunizations are given according to the Center of Disease Control (CDC).  We have the ability to bill through private insurance, after eligibility check is complete.  Children who have Medicaid, no insurance, or are underinsured (meaning insurance without vaccine coverage), American Indian or Alaskan Native are eligible for vaccinations under our VFC program.  The charge is $15.00 per administration.

Please call ahead for an appointment.  If the parent or guardian is unable to attend the appointment, please fill out PARENT PERMISSION FORM. (Parent permission form Spanish)   We will need to see insurance card and a form of ID at the visit, if you are unable to provide these items, your appointment will be rescheduled.

Birth to Age 18 Schedule

Children under 18 years of age must be accompanied by a parent or legal guardian or another adult over 18 years of age with our PARENT PERMISSION FORM, (Parent permission form  Spanish)  signed by the parent or legal guardian, along with a copy of the parents ID.  ID will be checked of named person accompanying the child.
