Back-To-School Immunizations

Staying up-to-date with immunization requirements for Indiana, in addition to taking advantage of recommended immunizations, can keep your child and our communities healthy to prevent and control transmission of vaccine-preventable diseases. This is in part why schools require back-to-school immunizations. After all, it is more difficult to treat a disease after an outbreak than it is to be proactive and prevent it with vaccines.

In-School Clinics

We are pleased to offer these clinics in an effort to help our busy parents.  Please watch for information on your schools website from the Dubois County Health Department.

In-school Tdap/ Meningitis immunization clinics for 5th grade students will offered in the winter months during school hours for all Dubois County schools students.

We are happy to announce we will be hosting a HPV and Hep A immunization clinic for 6th, 7th, and 8th grade students  in 2023-2024. The clinic will be offered in early fall months for dose 1, and early spring for dose 2.  HPV: Preventing Cancer One Vaccine at a Time!

In-school Meningitis/HPV/Hep A immunizations clinics for 11th grade students will be offered in the fall and winter months for dose 1 & 2, and early spring for dose 3 for all Dubois County students during school hours.

Immunization Late Night Clinics

In an effort to accommodate busy schedules the Dubois County Health Department will be offering late night immunization clinics the second Wednesday of each month until 5:30 p.m. for all vaccinations. Please call for an appointment 812-481-7056. Additional late night clinics are added as needed. 

Please Note: Bring a valid ID and insurance information. 

Children under 18 years of age must be accompanied by a parent or legal guardian or another adult over 18 years of age with our Parent Permission Form (English) Parent Permission Form (Spanish)  signed by the parent or legal guardian, along with a copy of the parents ID.  ID will be checked of named person accompanying the child. Updated insurance card is required at the time of appointment.  If you cannot provide these at your appointment, it will need to be rescheduled.
