Food Inspections & Complaints

The Health Department handles and investigates complaints on Food Establishments in Dubois County, Indiana.  If you have a complaint about an existing establishment, or to report an establishment that may not have a food permit, please submit this Complaint Form to the Dubois County Health Department or the electronic version on under Environmental Complaints tab.  All permitted establishments are inspected twice annually, or as needed based on risk and the amount of violations observed at time of inspection.
